JOB DESCRIPTION
Case Managers (CM) coordinate services promoting clients' independence in navigating daily living activities by empowering them to manage their symptoms. CMs complete work independently, utilizing case management training and following policies supporting best practices. CMs will have routine-to-moderate supervision but regularly consult with the CM supervisor on complex or challenging situations.
ESSENTIAL JOB FUNCTIONS
1. Conducts and documents a comprehensive assessment of the client's needs
2. Participates in developing the client's service plan, consistent with an assessment
3. Arranges for the delivery of needed services, as identified in the assessment
4. Assists clients in accessing services required through referrals or linkages
5. Monitors client's progress by tracking client appointments, performing follow-up on services rendered, and conducting
reassessments of the client's changing needs
6. Performs advocacy activities on behalf of the client (e.g., writing letters to expedite entitlement payments
and encouraging clients or family members to advocate on their behalf)
7. Prepares and maintains case records documenting contacts, services needed, reports, client's progress, etc.
8. Completes and uploads all required documentation into electronic health records and adapts to continually changing
documentation requirements
9. Provides case consultation (i.e., collaterally consulting with service providers to help determine the client's status and progress)
10. Collaborates with the supervisor to identify new resources and services and develop educational material for clients
11. Provides psychosocial education to clients and family members to help increase their understanding of the client's illness,
physical condition, or social situation
12. Performs crisis assistance (i.e., intervention on behalf of the client, planning for emergency referrals, coordinating other
needed emergency services)

EDUCATION PHYSICAL DEMANDS/REQUIREMENTS
• May involve 24-hour on-call responsibilities.
• Reliable transportation for frequent travel.
• Position requires lifting of 20 pounds maximum, with frequent lifting and carrying of items weighing up to 10 pounds
or requiring significant periods of walking or standing.
EDUCATIONAL REQUIREMENTS
• Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or a related field approved by KY Medicaid
or as required by applicable funding sources.
• Completion of state-required case management/service coordination course within six months of the date of employment.
SKILLS AND COMPETENCIES A successful Mandran Health CM will possess the following.
• Ability to identify the client's psychosocial needs by thoroughly completing relevant Strengths/Needs Assessments
• Establishes Treatment Plan objectives for the client based on activities that can address medical, social, educational, or vocational domains
• Identifies, networks with, and accesses community resources to address the specific needs of individual clients
• Knowledgeable of human growth and development stages, culture/ethnicity considerations, mental disorders,
and substance use disorder diagnoses relevant to age groups and populations served
• Manages caseload workflow to maximize client time or contacts to meet clients’ needs best
• Successfully manages multiple tasks within a set timeframe
• Maintains professional boundaries and operates within the scope of a CM, providing case management
• Finds merit in a job well done and enjoys problem-solving
