Case Manager
Case Managers (CM) coordinate services promoting clients' independence in navigating daily living activities by empowering them to manage their symptoms. CMs complete work independently, utilizing case management training and following policies supporting best practices. CMs will have routine-to-moderate supervision but regularly consult with the CM supervisor on complex or challenging situations.
